Japanese clinic hours follow patterns that are consistent nationwide once you know them, and baffling until you do.
The split shift
Most clinics run two blocks: roughly 09:00–12:30 and 14:00–18:00, closed in between. The gap is real — the door is locked and nobody answers the phone.
Wednesday and Thursday afternoons
Many clinics close for a half-day mid-week. Wednesday is the most common, Thursday next. It is a long-standing convention rather than a rule, but it is widespread enough that you should check before going on those days.
Saturday morning only
Saturday is usually morning-only, and Sunday is closed. Combined with the mid-week half-day, that gives a small practice something close to a weekend.
Reception closes before the clinic does
A clinic listed as open until 18:00 often stops accepting new patients at 17:30. For a first visit, allow even more time — you will have a form to fill in. Calling ahead is the only reliable way to know.
The exceptions
4,403 hospitals and clinics stay open until 20:00 or later on weekdays, and 6,744 open on Sundays. They exist, but you have to look for them.
